The request form collects your name, phone number, email address, ZIP code, Medicare situation, and any optional message you provide. It also records your contact permissions, the disclosure version and time, submission time, source page, initial landing page, referring website path, campaign parameters, and the review action used before submitting. Please do not submit Social Security numbers, Medicare numbers, medical records, or sensitive financial information.
How information is used
We do not sell leads. Website leads are used only to respond to Medicare inquiries and provide requested assistance, including arranging a conversation. Submitting this form does not apply for or enroll you in insurance. Contact consent is recorded separately from optional text consent.

Form field contents are not sent to analytics by this implementation.
Lead retention
We retain website lead information for 24 months after our last substantive interaction with you, then delete it or remove identifying details. If law requires a longer retention period for a specific record, we retain that record for the required period. This exception applies only to the affected records.
