Utah Medicare Guidance is the public facing Medicare business brand of Nicholas Security, which operates this website. References to “we,” “us,” and “our” in this policy mean Nicholas Security.
Information collected
The request form collects your name, phone number, optional email address, ZIP code, Medicare situation, and any optional message you provide. It also records your contact permissions, the disclosure version and time, submission time, source page, initial landing page, referring website path, campaign parameters, and the review action used before submitting. Please do not submit Social Security numbers, Medicare numbers, medical records, or sensitive financial information.
How information is used
We do not sell leads. Website leads are used only to respond to Medicare inquiries and provide requested assistance, including arranging a conversation. Submitting this form does not apply for or enroll you in insurance. Contact permission, nonmarketing text consent, and marketing text consent are recorded separately. We do not sell or share your SMS opt in data or personal information with third parties or affiliates for marketing purposes. Nonmarketing text messages cover requested assistance and appointment reminders; frequency varies. Marketing texts concern Medicare Advantage, Medicare Supplement, and Part D plan options, with up to 9 messages over 14 days. For subscribers to both, total frequency is up to 9 messages over 14 days. Message and data rates may apply. Reply STOP to opt out or HELP for help.
Storage and service providers
Requests are stored in the website’s server database. Leads may be accessed by Nick Catmull and service providers required to operate the website, CRM, messaging, or lead system. These providers support responding to your inquiry and providing requested assistance. When enabled, n8n and Telegram receive a limited notification for each accepted website request so Nick can respond promptly. No CRM destination is currently connected to this website.
Analytics and browser storage
This website uses session storage to retain campaign attribution, the initial landing page, the referring website path, the most recent review action, and a request confirmation within your browser session. Google Analytics 4 uses cookies and similar identifiers to measure visits, traffic sources, page views, downloads, and interactions such as successful form submissions and clicks on phone or appointment links. Meta Pixel measures page visits and accepted lead submissions to help evaluate Facebook and Instagram marketing. Form field contents, including your name, phone number, email address, ZIP code, and message, are not sent to these analytics tools by this implementation. Google Ads tracking is not currently configured. You can manage cookies through your browser settings.
How Google uses information from sites and apps
Email reminder signups
If you join the AEP reminder list, we store your email address, consent wording, consent date, and signup source. We use the address only for Medicare deadline reminders and coverage updates. The reminder list is separate from Medicare review requests and does not trigger a plan recommendation or enrollment. You may unsubscribe by replying to a reminder email or by contacting nick@utahmedicareguidance.com.
Lead retention
We retain website lead information for 24 months after our last substantive interaction with you, then delete it or remove identifying details. If law requires a longer retention period for a specific record, we retain that record for the required period. This exception applies only to the affected records. We may need to verify your identity and cannot delete a record while its legally required retention period applies.
